To ensure a high standard of care, space on our patient roster is limited. Completed applications go to our waitlist and we will notify you if space is available. Please note that your application does not guarantee a space in the practice.
Please do not inquire with our office regarding your position on the waitlist. Repeat inquiries may result in the lowering of your priority.
You will be randomly assigned to a doctor at enrollment. Requests for a specific doctor are not accepted. Our doctors provide comprehensive care inclusive of all ages, sex, race, and orientation.
